1. Guidelines still suggest a positive rapid strept or culture - even if high pretest probability; would never excede 50-75% confirmation. 2. According to guideline - Table 2 - maximum dose is 1 gram. 3. Can culture and treat - although data about relief of symptoms with antibiotics is minimal.

What the cephalexin vs. penicillin "controversy" -- ie. the empirical evidence (metaanalysis) suggesting lower failure rate with cephalexin?

Do adults over the age of 30 need to be treated with appropriate antibiotics(e.g. Penicillin/Amoxicillin) for a Rapid Strep positive test in order to prevent Acute Rheumatic Fever?
